We are seeking a highly skilled and passionate Program Worker to join our team at Family Connect and Support. As a Program Worker, you will play a critical role in providing high-quality, strengths-based, and person-centered intake, assessments, and interventions within a case management framework.
Key Responsibilities:- Provide timely information and advice to families and young people, as well as appropriate warm referrals and practical assistance to support their needs.
- Work collaboratively with key stakeholders, including families, carers, children, and young people, to build positive connections and achieve positive outcomes.
- Develop and maintain professional relationships with community resources and support services to ensure families and young people receive the support they need.
- Conduct assessments and interventions to identify and address the needs of families and young people, and develop and implement case plans to achieve positive outcomes.
- Work in a fast-paced environment, prioritizing tasks and managing multiple cases to ensure timely and effective support.
- Tertiary qualifications or equivalent experience in health, education, psychology, social work, social science, or social welfare.
- Demonstrated experience providing high-quality, evidence-based, customer-centered, culturally appropriate individual and/or family therapeutic interventions and referrals.
- Excellent written and oral communication, interpersonal, and negotiation skills, with the ability to communicate sensitively and effectively with all people.
- Demonstrated well-developed organisational, time management, and administrative skills, with the ability to plan, prioritise, and meet deadlines.
- Ability to identify and respond appropriately to child protection and other high-risk or vulnerability concerns.
