About the Role

We are seeking a dedicated and compassionate individual to join our team as a Case Manager. As a Case Manager, you will play a vital role in providing support and guidance to individuals who are experiencing homelessness or at risk of homelessness.

Key Responsibilities
  • Conduct in-depth client assessments to identify individual needs and develop personalized case plans.
  • Provide case management services to clients, including housing support, accommodation assistance, and connection to other community resources.
  • Collaborate with clients to develop and implement housing plans, ensuring they have access to safe and stable accommodation.
  • Facilitate the Accommodation Living Skills Program to equip clients with the skills and knowledge needed to maintain independent living.
  • Prepare and submit reports to the Team Leader or Manager as required.
Requirements
  • Social Work qualification or other relevant social tertiary qualifications.
  • Current and valid Victorian Employee Working with Children Check.
  • Current Victorian Driver's license.
  • Experience working with complex clients and ability to manage challenging behaviors.
  • Excellent communication and interpersonal skills.
