Job Description:

As a skilled Social Worker, you will play a vital role in promoting the social and emotional well-being of clients and their carers. You will work collaboratively with multidisciplinary teams to ensure coordinated and timely care for individuals with complex health needs.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments and develop tailored care plans that address the physical, emotional, and social needs of clients.
  • Provide expert support and guidance to clients and their carers, empowering them to make informed decisions about their care.
  • Work effectively with community-based services and other healthcare professionals to ensure seamless care transitions and optimal outcomes.
  • Develop and maintain strong relationships with clients, carers, and healthcare providers to facilitate effective communication and collaboration.

Required Skills and Qualifications:

  • Degree in Social Work from an accredited institution, providing eligibility for membership of the Australian Association of Social Workers.
  • Current Driver's Licence and willingness to use for work-related travel.
  • Demonstrated clinical expertise in assessment, care planning, and evaluation of social work interventions with adult clients who have complex health conditions.
  • Proven ability to work independently and as part of a team, with minimal direct supervision required.
