Job Summary:

We are seeking an experienced Clinical Care Coordinator to join our team at E4 Recruitment. As a Clinical Care Coordinator, you will be responsible for managing and coordinating clinical care services to provide and maintain excellent care for our residents. Key Responsibilities:

  • Supervise the delivery of care services to ensure they meet safety, effectiveness, and regulatory standards.
  • Lead, mentor, and support the nursing team, including managing schedules, conducting performance reviews, and fostering a positive work environment.
  • Develop and review individualized care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
  • Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
  • Ensure accurate and timely documentation of care plans, incidents, and resident records.

Requirements:

  • Prior experience as a Clinical Care Coordinator (or similar position) is preferred but not essential.
  • You must be an experienced Registered Nurse with a current AHPRA registration and a strong desire to expand your career in aged care.
  • Strong clinical knowledge, leadership, organisational, and communicative skills are essential for this role.
  • Excellent knowledge and experience of aged care accreditation, regulations, standards, and best practices are required.
