**About the Role**This position presents a valuable opportunity to make a positive impact in the lives of individuals with physical and mental disabilities.You will provide support to clients across Perth region as part of our team, facilitating the delivery of quality services and achieving positive outcomes with service users in a positive and proactive environment.

- Support participants to work towards achieving their individual goals.

- Assist with daily living tasks such as cooking, housekeeping, hygiene, shopping, medication administration and community access.

- Foster participant confidence and independence.

- Liaise with clients, community and relevant agencies to ensure collaborative support.

- Monitor progress against support plans.

- Utilise positive behaviour strategies to promote participant well-being.

- Participate in lifestyle planning and review outcomes.

- Work with clients, family/carers, case managers and professionals to achieve individual goals.

- Collaborate with colleagues to ensure smooth running of disability services.

- Report regularly to management.

**Required Skills & Qualifications**

- Qualification in community/disability/individual support/aged care/mental health or equivalent experience.

- Experience working with clients with challenging behaviours.

- Well-developed verbal and written communication skills.

- Current driver's licence and reliable vehicle.

- 100-point ID check.

- Valid visa/work rights in Australia.

- Possession of, or willingness to obtain, necessary certifications prior to commencement:

- First aid/CPR.

- Medication certification.

- Manual handling certification.

- Working with Children Check.

- NDIS clearance check.

- NDIS orientation module certification.

- Covid-19 vaccination evidence.

- Hand hygiene certification.

- Covid-19 infection control certification.
