About the Role
- We are looking for a highly skilled Clinical Pharmacist with a passion for delivering exceptional patient care and promoting quality use of medicines.
- The ideal candidate will have minimum 1 year of experience as a Registered Pharmacist in an Australian-based Pharmacy, with strong communication and collaboration skills.
- You will work closely with our multidisciplinary team to provide hands-on clinical pharmacy services, supporting patient care and medication management.
Key Responsibilities:
- Conduct medication chart reviews and reconciliation, ensuring accurate and safe medication administration.
- Dispense and supply medication and pharmacy products, maintaining a high level of customer service.
- Coordinate patient medication requirements, including inpatient stays, day infusions, and discharge planning.
- Develop and maintain professional relationships with key stakeholders, promoting excellent customer service.
- Educate and advise staff on health promotion, disease prevention, drug selection, and quality use of medicines.
