Job Title

Care Coordinator Leader

We are seeking a dedicated leader to join our care team, providing holistic support to residents in their individual needs, wishes, and goals.

Key Responsibilities:
  • Ensure comprehensive social and emotional support to all residents.
  • Coordinate and manage the care requirements of residents across different areas.
  • Lead and supervise care teams on shift, ensuring effective management of medications, nutrition, and hydration.
  • Perform tasks related to medication management within the scope of qualification and regulatory requirements.
About the RoleRequirements:
  • Diploma of Enrolled Nursing (with AHPRA Registration) or Certificate IV in Ageing Support.
  • High level of integrity, honesty, and trustworthiness.
  • Effective communication skills with ability to communicate at all levels.
  • Ability to use information technology and programs effectively.
  • Current National police check and working knowledge of WH&S and Infection Control.
