About This Role

We are seeking a qualified, reliable and passionate administrator (scheduler) to join our team. The successful candidate will coordinate, manage and administer the delivery of care services to our clients with professionalism and dedication.

Main Responsibilities
  • Conduct comprehensive client care assessments and create personalized care plans that meet their needs.
  • Manage client compliance and a case load of clients with attention to detail and a focus on delivering excellent outcomes.
  • Process intakes, develop clinical care plans and report results accurately and efficiently.
  • Cultivate partnerships with clients, families and healthcare professionals to ensure seamless care coordination.
  • Perform general administration tasks with precision and speed.
  • Participate in on-call rotations with the care management team to provide 24/7 support.
  • Respond to client inquiries via phone and in-person with empathy and expertise.
Requirements
  • A minimum of Certificate 3 in Aged Care/Individual Support or equivalent qualification.
  • A valid driver's license for travel to client locations.
PREFERRED SKILLS
  • Previous experience in aged care, NDIS or mental health care coordination roles.
  • Knowledge of My Aged Care, Home Care Package and NDIS regulations.
  • Enrolled Nurse or Registered Nurse qualifications an advantage.
  • High level of computer literacy, including proficiency in Microsoft Office applications.
  • An outgoing personality with high energy levels, excellent communication skills and a friendly telephone manner.
  • Ability to solve problems quickly under pressure and maintain a calm demeanor.
  • A positive attitude, ability to work collaboratively in a team environment and multitask effectively.
  • Excellent networking, consultation and customer service skills.
