The position is accountable to the Clinical Manager for contribution to the implementation and management of the service delivery model. The incumbent will be involved in actively discharge planning, coordination of client services, provision of Social Work assessment and intervention, and contributing to reporting and quality monitoring mechanisms.

About the Role

">

Reporting to the Clinical Manager, this role contributes to the implementation and management of the service delivery model and Transition Care Program. Duties include:

  • ">
  • Effectively plan and implement Social Work assessment and intervention skills within the service.">
  • Undertake timely and evidence-based Social Work assessments, develop SMART goals, and provide appropriate levels of Social Work intervention.">
  • Work collaboratively with hospital staff and family members to ensure patient-focused TCP program referrals and assessments are conducted according to guidelines.">
  • Contribute to a multi-disciplinary team to continually improve, implement, and evaluate the holistic Transition Care program responsive to community needs.">

About You

">

The ideal candidate has an appropriate degree or equivalent qualification giving eligibility for full membership of the Australian Association of Social Workers. Demonstrated skills in case management, care coordination, and complex clinical decision-making are required.
