Job SummaryWe are seeking a skilled Community Registered Nurse to join our Home Care program in Bendigo. As a key member of our team, you will provide clinical assessments, reviews, and care needs for clients in the East and Outer Eastern suburbs. Your expertise will enable you to provide collaborative leadership, support, and supervision to Care Advisors and Home Care staff.
Key Responsibilities- Conduct in-home clinical assessments, reviews, and care needs for clients
- Provide clinical advice and support to clients, and refer them to services where specialized support is required
- Lead and support Care Advisors and Home Care staff in delivering high-quality care
- Collaborate with local allied health professionals to facilitate responsive care in clients' homes
- Develop and implement health education and promotion programs, including social support groups and carer education
- Promote a harmonious team approach by modeling safe practice measures and achieving clinical support goals
- Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration
- Minimum 3 years of experience as a Registered Nurse in Community, Commonwealth Home Support, or District Nursing
- Strong team player with a positive outlook and flexible approach to work
- Endorsed for Continence Assessments and Prescription (highly desirable)
- Experience in visiting clients in their homes and evaluating and implementing care support plans
- Familiarity with the My Aged Care portal and proficiency in decision-making, critical analysis, and problem-solving
- Excellent communication and delegation skills, with the ability to prioritize in a fast-paced environment
- A current Working With Children Check card
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance and NDIS Clearance
