About the Role
This is an outstanding opportunity for a highly motivated and experienced healthcare professional to join our team as a Clinical Care Manager. As a key member of our leadership team, you will be responsible for ensuring that all residents within our facilities receive the highest quality of clinical care.
The successful candidate will possess strong clinical expertise and leadership skills, with the ability to provide guidance and support to our care teams. You will also be responsible for managing and documenting assessments, care plans, and AN-ACC requirements in accordance with best practice clinical standards.
Key Responsibilities
- Provision of clinical expertise and leadership to care teams across our facilities.
- Assist the Care Manager to lead carer teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators.
- Responsible for managing all aspects of assessment, documentation, and submission of AN-ACC requirements.
- Support the effective documentation and user support of policies, procedures, and IT system requirements.
- Coordination of assessment, care planning, and review processes.
- Support the admission of new residents and ensure accurate information is recorded.
- Input and oversight into facility team rosters and ensure optimal skill mix is available.
- Manage and document clinical risk and ensure a commitment to continuous improvements.
