Home Health Patient Care Manager Opportunity

We are seeking a skilled and compassionate Home Health Patient Care Manager to join our team. In this role, you will play a vital part in delivering high-quality patient care services in patients' homes.

As a Patient Care Manager, your responsibilities will include:

  • Providing clinical leadership and supervision to healthcare teams to ensure delivery of exceptional patient care.
  • Coordinating patient assessments, care planning, and interventions to meet individualized patient needs.
  • Managing patient referrals, assigning clinicians, and evaluating patient orders to initiate start-of-care visits.
  • Ensuring timely and accurate documentation of patient information, including medical necessity determinations and insurance approvals.
  • Collaborating with interdisciplinary teams to assess patient needs continually and ensure that care is individualized, appropriate, and meets eligibility criteria and physician orders.

Requirements for success in this role include:

  • Current RN licensure and CPR certification.
  • Valid driver's license and access to reliable transportation.
  • Strong leadership, communication, and problem-solving skills, with the ability to work effectively in a fast-paced environment.

Key responsibilities and expectations of this role include:

  • Supervising and coordinating clinical services, providing and directing nursing care to patients in their homes as prescribed by the physician, in compliance with applicable laws, regulations, and agency policies.
  • Ensuring compliance with all relevant regulatory requirements and standards, including HIPAA and OSHA guidelines.
