About the Role

Lutheran Services provides vital support to vulnerable members of our community. As a Support Worker, you will be an integral part of this mission, delivering essential services in clients' homes and communities.

Key responsibilities include:

  • Delivering person-centred care, supporting clients with daily tasks
  • Conducting client assessments, identifying goals, interests and needs
  • Collaborating with service coordinators, ensuring effective communication and carer involvement
  • Maintaining risk mitigation procedures and reporting new risks
