Job Summary

We are seeking a highly skilled and passionate Senior Care Coordinator to deliver exceptional services. This role focuses on enhancing all aspects of care, ensuring our residents achieve the highest quality of life with an emphasis on legislative requirements.

This position involves supervising and managing the care team, monitoring residents' needs, and participating in case conferences to evaluate care plans as per policy.

The key responsibilities include:

  • Leading the care team to provide high-quality care that enables residents to have the best possible life
  • Monitoring all residents' care needs to meet their preferences, considering cultural, religious or ethnic backgrounds
  • Participating in case conferences and evaluating care plans according to policy

We are looking for a candidate with current registration as a Registered Nurse with AHPRA, minimum 5 years post-graduate nursing experience, and strong clinical background in wound care and palliative care.

The ideal candidate will also possess comprehensive knowledge of clinical issues related to ageing, including dementia-related disorders and the management of challenging behaviours, and working knowledge of the Australian National Aged Care Classification (AN-ACC) system.

Required Skills and Qualifications

To be successful in this role, you will need:

  • Current registration as a Registered Nurse with AHPRA
  • Minimum 5 years post-graduate nursing experience
  • Strong clinical background in wound care and palliative care
