About Care Management

The role of a care manager is to oversee the delivery of healthcare services to seniors and individuals with disabilities, enabling them to maintain their independence and age gracefully in their own homes.

We employ our own ERP and CRM software to operate more efficiently than our competitors, passing on these savings to our clients. Unfortunately, too much of our clients' budget goes towards administrative tasks rather than direct healthcare provision.

We have experienced significant growth over the past three years and are seeking an additional care manager to support our expansion plans and drive success to the next level.

About The Role

This is an exciting opportunity for a qualified and experienced healthcare professional to join our team as a care manager. Your key responsibilities will include managing client issues, supervising care delivery, and reviewing progress notes.

  • Key Responsibilities:
  • Composing Client Assessments and Plans
  • Managing Client Issues
  • Supervising Care Delivery
  • Reviewing Clients Progress Notes and Ensuring Corrective Actions Are Implemented
  • Liaising with Clients/Primary Contacts to Ensure Service Expectations Are Being Met and Issues Are Discussed Before Complaints Arise
  • Working with the Scheduling Team to Ensure Staff and Clients' Expectations Are Met
  • Chairing Regular Team Meetings with Staff in Order to Support Them in Their Day to Day Duties
  • Conducting Performance Reviews with Your Team
