We are seeking a skilled professional to join our multidisciplinary health team as a Care Coordinator. This role involves supporting patients and their families within the aged care wards, delivering services including crisis intervention, psychosocial assessments, general counselling, advocacy, case management, therapeutic and grief/loss counselling, and complex discharge planning.
The ideal candidate will have a degree in Social Work with eligibility for membership of the AASW, and demonstrated understanding of the psycho-social impact of unexpected hospitalisation due to illness, accident or trauma.
