About the Role

We are seeking an experienced Clinical Care Coordinator to join our residential care team at our St George Care Centre in Bayswater. As a key member of our team, you will be responsible for leading a team of care staff, including Registered nurses, Enrolled nurses, and carers, while contributing to a positive workplace culture.

Key Responsibilities
  • Supervise the daily delivery of care to residents by the care team
  • Provide resident advocacy and coordinate assessments and documentation for residents
  • Ensure accurate care planning and relevant supporting documentation are provided in a timely manner
  • Provide relevant education, coaching, and support to the team
  • Drive and monitor ANACC assessments and process to maximise funding
  • Investigate and manage clinical issues and client complaints
  • Conduct and delegate clinical audits and lead actions to close identified clinical gaps
  • Ensure the rostered direct care staff are appropriately allocated to provide the highest levels of care to the resident
  • Contribute to the performance appraisal process for all staff within area of responsibility
  • Ensure care consumables are managed responsibly and within budget
Requirements
  • Valid Police Clearance
  • NDIS Clearance certificate
  • Evidence of flu and covid vaccinations
  • Ability to pass a pre-employment medical and reference checks
  • Registration with AHPRA for a minimum of 5 years
  • High level of clinical expertise and currency of practice
  • Proven organisational and time management skills
  • Excellent interpersonal communication skills
