Job Overview

We are seeking a skilled Community Care Specialist to join our multidisciplinary team. As a registered nurse, you will deliver high-quality care in the community setting, working closely with clients and their families.

Key Responsibilities
  • Deliver safe, reliable, and responsive client-based care in the community environment.
  • Contribute to comprehensive assessment, care planning, and treatment to meet clients' clinical needs.
  • Work independently and collaboratively within a multidisciplinary team environment.
  • Problem-solve and make decisions in complex situations.
Requirements
  • Minimum of three years acute clinical and community nursing experience as a Registered Nurse.
  • Passion for delivering exceptional patient care.
  • Experience in wound management, CVADs, palliative care, negative pressure wound therapy, stoma, and catheter care.
