Community Care Nurse Position

We are seeking a skilled Community Care Nurse to join our team in providing high-quality patient care within the community setting.

The ideal candidate will have a strong background in community nursing, with experience in wound management, CVADs, palliative care, and other relevant areas. They will also possess excellent communication and problem-solving skills, as well as the ability to work independently and as part of a multidisciplinary team.

Job Description:

The Community Care Nurse will be responsible for delivering client-based care to individuals in their homes, focusing on safe, reliable, and responsive care. This will involve assessing client needs, developing care plans, and implementing treatment strategies to ensure optimal outcomes.

Required Skills and Qualifications:
  • A minimum of three years acute clinical and community nursing experience as a Registered Nurse
  • Passion for community nursing
  • Experience in wound management, CVADs, palliative care, and other relevant areas
  • Ability to work independently and without direct supervision within an MDT environment
  • Ability to problem solve and make decisions in complex situations
  • Commitment to ongoing professional development
