We are seeking a highly motivated and experienced Support Coordination and Psychosocial Recovery Coach to join our team at SWISS. As a Support Coordinator/Recovery Coach, you will be responsible for delivering NDIS funded provision of Support Coordination and Psychosocial Recovery Coaching services to participants in the South West, Western Australia region.
Key Responsibilities:
- Support participants to understand all aspects of their NDIS plan and navigate the NDIS provider network.
- Assist participants to source and implement required services and supports to facilitate progression towards achieving their goals.
- Network with government, community, and mainstream services to refer participants, promote NDIS services, and increase new participant referrals in our region.
- Monitor plan budgets and support effectiveness, and design creative and innovative approaches to encourage individual skills development.
Requirements:
- Local to the South West, Western Australia region.
- Minimum Qualification in Mental Health, Disability services, and/or 2yrs+ of lived or work experience in mental health/psychosocial disability.
- Expertise across a diversity of disability types, including psychosocial disability and recovery-oriented practice.
- Rights to work in Australia.
- A current valid Driver's License.
- National Police Records Check [less than 12 months old].
- NDIS Workers Screening Check.

Pay: $42.00 – $55.00 per hour
