Community Care Specialist

Deliver high-quality nursing care in a community setting, utilizing best practice evidence-based resources. Travel across the Toowoomba region to provide comprehensive assessments and case management to clients in their homes.

  • Conduct thorough client assessments, documenting current status and requirements while adhering to relevant policies and procedures.
  • Develop holistic care plans from assessments, ensuring effective delivery of services.
  • Communicate effectively with clients, families, and healthcare providers to ensure ongoing care development and review.
  • Regularly review client progress against care plans, modifying as needed based on changing care needs.
  • Liaise with colleagues and other healthcare service providers involved in client care plans.
  • Deliver care in a safe and competent manner, adhering to professional standards.
  • Document care provided according to organisational guidelines and client management systems.
  • Coordinate, supervise, and monitor delivery of care by care staff, delegating tasks appropriately.
  • Administer medications as prescribed, adhering to relevant legislation and medical orders.
Requirements:
  • Current National Police Certificate.
  • Current Driver's Licence and reliable vehicle.
  • Current CPR Certification.
  • Degree in Nursing or equivalent qualifications and experience.
  • Current AHPRA registration and evidence of recent practice.
  • Experience working as a Registered Nurse in community aged care.
  • Ability to coordinate and deliver quality nursing care within risk management frameworks and professional standards.
  • Effective time management and problem-solving skills.
