About the Opportunity

We are seeking a professional to fill a key role in our organisation.

As a Case Manager, you will deliver support and develop programs for relapse prevention, harm reduction, and well-being.

Responsibilities
  • Deliver residential support services
  • Develop community-based programs
  • Contribute to resident plan formulation and review
  • Build relationships with clients and their families
  • Ensure resident comfort and dignity
  • Perform assessments and intakes as required
  • Participate in program content development
  • Maintain accurate case notes and documentation
Requirements
  • A Certificate IV in Community Services (Alcohol and Other Drugs) or a related qualification
  • Experience working in case management
  • Experience in AOD or therapeutic community settings
  • Experience designing, facilitating and participating in group programs
  • Excellent communication skills, with the ability to engage with diverse clients and stakeholders
  • The ability to maintain accurate records and deliver goal-oriented support
  • A sound understanding of the TC model and the ability to respond effectively to challenging behaviours
  • Strong organisational, time management, and documentation skills
  • Lived experience and/or cultural competence highly desirable
