We are seeking a highly skilled and experienced Registered Nurse to join our GEM/REHAB@HOME team as a Nurse Practitioner. As a key member of our multidisciplinary team, you will provide high-quality nursing care to patients in their own homes, working closely with Geriatricians, Specialist Rehabilitation Physicians, and other healthcare professionals.
Key Responsibilities- Provide comprehensive nursing care to patients in their own homes, including medication management, wound care, and assessment.
- Work collaboratively with the multidisciplinary team to coordinate and provide expert clinical services.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct regular assessments and evaluations to ensure the highest level of care and quality of life for patients.
- Minimum of 5 years acute medical/surgical post-registration nursing experience or nursing specialty equivalent.
- Current Division 1 Nursing registration with AHPRA.
- Demonstrated initiative and self-direction.
- Willingness to maintain and advance knowledge and skills relevant to this position.
- Comprehensive systems-based patient assessment skills and documentation of findings.
- Working knowledge of wound management (acute and chronic).
- Urinary catheterization - male and female.
- Pathology result interpretation.
- A strong commitment to high-quality care.
- The ability to work as part of a diverse team.
- Excellent communication and organizational skills.
- Current Victorian Driver's License.
