  • Support clients with daily living tasks, such as personal care, meal preparation, and light housekeeping.
  • Assist clients with accessing local services and resources, including healthcare, transportation, and social activities.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless service delivery.

Requirements:

  • Diploma or degree in a relevant field (e.g. aged care, nursing, health science).
  • Minimum 2 years' experience working in a similar role.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's licence and reliable vehicle.
