This role supports Aboriginal and Torres Strait Islander individuals with severe mental illness to access care during their transition from custody to the community.
About the RoleThe clinician will collaborate with custodial mental health services, primary health networks, and other organisations to provide continuity of care. This includes developing mental health plans, establishing necessary therapeutic and cultural supports, and providing follow-up community care.
Key Responsibilities- Work in a multidisciplinary team to support Aboriginal and Torres Strait Islander people with severe mental illness.
- Develop mental health care plans and establish necessary therapeutic and cultural supports.
- Provide follow-up community care to individuals leaving custody.
- Collaborate with custodial mental health services, primary health networks, and other organisations to ensure continuity of care.
- Be of Aboriginal or Torres Strait Islander descent.
- Hold a Bachelor degree or higher in a relevant field such as health science, psychology, or social work.
- Have current registration with AHPRA for Occupational Therapy or Psychology professions, or eligibility for membership of the Australian Association of Social Workers (AASW) for Social Work.
- Have at least three years' full-time equivalent post-registration experience in mental health, and an approved post-registration tertiary qualification in mental health.
