Job Description

Purpose & People are seeking a Behaviour Support Practitioner to join their team. As a key member of the support team, you will play a vital role in helping clients find fantastic people to join their teams.

We work closely with our clients, preparing them to put their best foot forward and shine before they meet with their potential new boss.

 

About Our Client

Our client is a Victorian-based provider specialising in working with participants with complex care requirements. They empower their clients and families to have maximum choice and control in their lives through Supported Independent Living and Short-Term Accommodation houses.

 

About Your New Role

We are looking for a Behaviour Support Practitioner to join their expanding team. You will deliver high-level, evidence-based clinical advice and assistance to participants, their families, and key stakeholders.

Through the creation, implementation, and education of positive behaviour support plans, your person-centred approach will focus on reducing the impact of challenging behaviours to enhance their quality of life.

  • Work with your own caseload, assisting individuals and their families to identify their needs through assessment and tailored positive behaviour support planning.
  • Work as part of a trans/multi-disciplinary model with a range of professionals to provide a holistic and integrated approach to support.
  • Liaise with and develop strong, collaborative relationships with external stakeholders, government, and non-government service providers for ongoing support.
