About the Role

We are seeking a passionate and dedicated Support Worker to join our team at New Horizons. As a Support Worker, you will play a vital role in providing support to individuals with disabilities, promoting their independence, and enhancing their quality of life.

The successful candidate will possess a positive, authentic, and inclusive personality, with excellent communication and interpersonal skills. You will be required to work in a variety of settings, including supported independent living and inclusion services, and will be expected to maintain professional relationships with individuals, families, caregivers, and key stakeholders.

Key Responsibilities:

  • Providing support to individuals to improve their independence and daily living skills
  • Assisting with transportation, social activities, medication, and personal care
  • Developing and maintaining professional relationships with individuals and stakeholders
  • Completing risk assessments, support plans, and other required documentation

Requirements:

  • Appropriate certificate-level qualification in Disability or Community Services, or equivalent relevant experience
  • Valid NSW Drivers C class license and ability to drive vehicles in license class
  • Ability to lift 10 kilograms unassisted and undertake physical requirements of customer support
  • Intermediate skills in Microsoft Office and other common systems
