About the Role

BaptistCare is seeking a skilled Care Facilitator to join our team in the Bathurst region. As a Care Facilitator, you will play a vital role in supporting clients to live well in their own homes.

Key Responsibilities

  • Visit clients in their homes to assess their needs and develop individualized care plans
  • Work closely with clients, families, and carers to set and achieve goals that promote independence and well-being
  • Participate in the development and implementation of services and programs that meet clients' needs
  • Document client information and progress using client record management systems

Requirements

  • Bachelor's Degree or Diploma in Community Services or equivalent
  • Valid driver's licence
  • Intermediate level of computer skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
