Key Responsibilities- Provide case management services to individuals living with a disability and/or mental illness, ensuring that their needs are met and their goals are achieved.
- Develop and implement individual support plans, utilizing evidence-based practices and working collaboratively with clients, families, and other stakeholders.
- Provide one-on-one support, including medication assistance and basic personal care, to clients in their homes or in our centre.
- Maintain accurate and confidential records, ensuring that all documentation is up-to-date and compliant with relevant regulations.
- Work collaboratively with other team members to ensure that clients receive comprehensive and coordinated support.
- Tertiary qualifications in a relevant field, such as social work, psychology, or disability studies.
- Experience working with individuals living with a disability and/or mental illness, preferably in a case management or support worker role.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and other stakeholders.
- Ability to work independently and as part of a team, with a high level of flexibility and adaptability.
- Current Australian driver's license and a willingness to travel to clients' homes or other locations as required.
