Clinical Partnership Opportunity

Overview:

  • An experienced clinical partner to support a team in Northern Sydney.

This role involves providing evidence-based, best practice clinical care holistically to individuals, ensuring their unique goals, needs and preferences are understood and addressed.

About the Role
  • Assessing clinical care needs is an essential aspect of this role.
  • Developing and reviewing care plans will be key to delivering high-quality patient outcomes.
  • The ideal candidate will have excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.

This role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care.

Requirements
  • Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General)
  • Passion for and previous experience in client-focused service delivery in aged and community/home care settings
  • Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards
  • Excellent administrative and computer skills, including digital health records and care coordination tools

The ideal candidate will have a strong understanding of clinical care principles and be able to work effectively in a multidisciplinary team.
