About the Opportunity
  • Deliver expert clinical care and select therapeutic interventions through independent decision-making, preventing hospital admissions through home-based care delivery.
  • Provide short-term case management to clients in Adelaide's southern suburbs, ensuring effective complex discharge planning and hospital avoidance.
  • Link clients and carers with relevant community resources and services, promoting seamless transitions and improving patient outcomes.
  • Communicate effectively with clients' family, carers, and other service providers, fostering strong relationships and collaborative working practices.
About You
  • RN3: Registered Nurse with 3+ years post-registration experience, demonstrated competence in geriatrics and community Nursing practice.
  • AHP3: Hold an appropriate degree-level qualification, membership (or eligibility) with a relevant professional association, and/or registration with the relevant Board.
  • Demonstrate ability to assume responsibility for clinical allied health services as a member of an interdisciplinary team.
