We are seeking a highly skilled Clinical Funding Officer to join our team at Southern Cross Care QLD. As a Clinical Funding Officer, you will play a critical role in providing site-specific clinical funding support to our residential aged care homes.
Key Responsibilities- Active involvement in admission processes, including pre-screening, AN-ACC funding predictions, and commencement of assessment and care plans.
- Monitoring and tracking of residents' data, including movements, Medicare events, and AN-ACC lodgements.
- Reviewing assessments to ensure congruency and accuracy in care planning.
- Delivering high-quality documentation, reporting, and tracking.
- Ensuring occupancy, entry, and departure monitoring.
- Completing allocated KPIs for resident screening and reclassification requests.
- Experience in a care or clinical role within a residential aged care facility.
- Knowledge and understanding of AN-ACC tools, AN-AC funding guide, and processes.
- Strong written and communication skills.
- Proven ability to quickly acquire knowledge of diverse software systems.
- Effective time management skills.
- Ability to work as part of a team and with minimal supervision.
