The role of Service Facilitator is a dynamic position where you will work with clients and staff in the community and in their homes, coaching and mentoring staff, and working collaboratively across teams to ensure exceptional customer service in the Brisbane South/Gold Coast region.
">OverviewAs a Service Facilitator, you will be responsible for implementing, managing and maintaining individual client goals to provide high-quality services and supports. You will also manage rosters daily to ensure accuracy and timely claims for payment.
You will supervise, coach and mentor direct care staff to enhance their performance and ensure effective communication and coordination between services, staff and stakeholders providing support to clients.
Additionally, you will assist clients in managing finances according to CPL policies and procedures, and Public Trustee guidelines when applicable. You will also maintain a good understanding of relevant Aged Care legislation to protect vulnerable individuals from abuse and neglect.

- Experience in a similar Aged Care role, HCP and CHSP.
- Tertiary qualifications in health are desirable.
- Commitment to delivering the highest level of customer service.

- Ability to work autonomously and collaboratively within teams.
- Well-developed administrative and financial management skills.
- Current C class driver's license and reliable transport.
- Current First Aid and CPR certification.
- Competency in Microsoft Office and computer systems.
- Working With Children Check and NDIS Worker Screen Clearance (CPL can assist).
