Support Worker Role

We are seeking a dedicated and compassionate individual to join our team as a support worker. In this role, you will have the opportunity to make a meaningful difference in the lives of individuals with disabilities.

About the Role:

As a support worker, you will be responsible for providing support and guidance to our participants and their families. This may include conducting assessments, developing person-centred plans, and evaluating the effectiveness of support plans.

  • Key Responsibilities:
  • Conduct comprehensive assessments to identify the needs of NDIS participants.
  • Develop and implement person-centred plans that reflect the goals, passions, and strengths of NDIS participants.
  • Evaluate and improve the effectiveness of support plans, ensuring they align with best practice.
  • Work collaboratively with multidisciplinary teams to deliver high-quality support services.
