
Care Coordinator

Become a vital part of our team and support older people to live well in the comfort of their own home. This role is ideal for individuals who are passionate about empowering clients to live independently and improve their quality of life.
As a Home Support Worker servicing clients in rural areas, you will play a key role in assisting clients and their families to engage in services, enabling them to remain living in their home. You will develop individualised care plans and work closely with clients to set achievable goals focused on improving their well-being.
Key Responsibilities:- Visit clients in their homes and provide person-centred care to support their independence
- Work collaboratively with multidisciplinary teams to deliver high-quality services and programs that meet clients' needs
- Develop strong relationships with families and carers to understand their concerns and needs
- Assess clients' needs and create individualised care plans to achieve desired outcomes
- Document client interactions and progress in a timely and accurate manner
This role requires flexibility, excellent communication skills and a compassionate nature. You will need to be comfortable working independently in the community and have intermediate computer skills.
RequirementsTo be successful in this role, you will need:
- A Bachelor's Degree or Diploma in Community Services or equivalent qualification
- Intermediate level of computer skills
- Excellent time management and organisational skills
- Proven experience working with aged care or similar populations
