Community Health Practitioner

We are seeking a skilled Community Health Practitioner to deliver vital health care services to patients, families and carers in a supportive multidisciplinary healthcare team. As a valued member of our team, you will work in a respected health district making real community impact.

Key Responsibilities
  • Provide social work services to patients, families and carers of the hospital and local health district.
  • Exercise independent professional judgement on routine matters.
  • Assist in the development of policies, procedures, standards and practices.
  • Participate in quality improvement activities, departmental committees and clinical research activities.
Essential Requirements
  • Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
  • Demonstrated clinical experience in discharge planning, NDIS, child protection, domestic violence, grief and bereavement.
  • Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client groups.
  • Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills.
  • Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
  • Knowledge of quality assurance and/or clinical practice improvement activities.
  • Ability to participate in the After Hours Crisis On Call Social Work Service.
