About the Role
We are seeking a dedicated and compassionate Nurse Care Coordinator to join our team at Regis Aged Care. As a key member of our clinical team, you will be responsible for delivering high-quality care to our residents, working collaboratively with our carers and other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities
- Lead exceptional care outcomes through the coordination and delivery of person-centred care.
- Oversight of the resident across the day, identifying changes in preference, needs or wellbeing, and prompt escalation for support.
- Develop comprehensive care plans, conduct thorough assessments of needs and preferences, and implement appropriate care strategies.
- Manage and safely administer medications following established protocols.
- Maintain accurate and up-to-date documentation, including admission assessments, care plans, interventions, and progress notes.
- Work in partnership with our carers to deliver our Continuity of Carer model.
