About the Role
As a Nurse Care Coordinator, you will lead exceptional care outcomes through the coordination and delivery of person-centred care.
You will oversee the resident across the day, identifying changes in preference, needs or wellbeing, and prompt escalation for support.
You will see the resident as an individual, prompting choice and independence, with support provided as needed.
You will develop comprehensive care plans, conduct thorough assessments of needs and preferences, and implement appropriate care strategies.
You will manage and safely administer medications following established protocols.
You will maintain accurate and up-to-date documentation, including admission assessments, care plans, interventions, and progress notes.
What will you bring
- A passion for caring for others.
- Commitment to a holistic care approach and supporting residents to achieve their goals and support their needs.
- Bachelor of Nursing degree or recognised equivalent qualification.
- Current AHPRA Registration (Nursing).
- Understanding or ability to learn the attributes of the AN-ACC funding system.
- Experience in complex nursing clinical services and/or aged care setting, hospitals, or community care.
