About The Role

This key member of our team supports the Clinical Care Coordinator and Registered Nurse to oversee and coordinate care, ensuring each resident's individual needs are met in a supportive environment.

Key Responsibilities
  • Deliver holistic, person-centered care by providing social and emotional support to all residents.
  • Support coordination of care needs across the home, including medication management, nutrition, hydration, and pressure care management.
  • Lead/supervise care on shifts, within designated wings or areas.
