We are seeking a dedicated and compassionate Disability Support Professional to join our team at NGARE EMPLOYMENT SOLUTIONS PTY LTD. As a key member of our team, you will play a vital role in supporting individuals with disabilities to achieve their goals and live fulfilling lives.
Key Responsibilities- Provide personalized support to clients with disabilities, promoting independence and autonomy.
- Develop and implement individualized support plans, working closely with clients, families, and other stakeholders.
- Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
- Conduct regular assessments and reviews to monitor client progress and adjust support plans as needed.
- Maintain accurate and up-to-date records of client interactions and support plans.
- Full Australian working rights.
- Current unrestricted WA Drivers Licence.
- Current Provide First Aid & CPR Certificate or willingness to obtain prior to commencing.
- Valid NDIS Worker Screening Check.
- Own motor vehicle which is roadworthy, registered and insured.
- Previous experience working in Disability or Aged Care (desirable).
- Certificate in individualized support, disability and/or aged care (desirable).
