Job Opportunity:

The Community Staff Nurse will play a pivotal role in supporting skill mix teams across Wakefield, collaborating closely with School Nurses and Family Health Practitioners to deliver the Healthy Child Programme.

Key Responsibilities
  • Contribute to targeted interventions and health promotion as directed by the Specialist Community Public Health Nurse (SCPHN)
  • Coordinate, plan, implement, and evaluate a programme of care for children, young people, and families
  • Support comprehensive health needs assessment and mobilize resources to meet identified needs
  • Promote inter-agency partnership working to facilitate integrated approaches in meeting child needs

Excellent communication skills, passion for public health, and good organizational skills are essential. Ability to travel independently and flexibility to meet service needs is required.
