  • Collaborate with clients, their families, and a multidisciplinary team to develop person-centered care plans and ensure holistic support
  • Deliver individual and group therapy sessions to enhance daily living skills, cognitive abilities, mobility, and self-care
  • Maintain accurate and up-to-date client records and documentation
