Job Title: Home Care Professional

**Job Summary:**We are seeking a skilled Home Care Coordinator to join our team. The ideal candidate will have excellent communication and coordination skills, with the ability to work effectively with clients and healthcare professionals.**Key Responsibilities:**
  • Deliver efficient and effective client-focused care coordination of eligible Commonwealth Home Support Program clients.
  • Undertake effective case planning and management through client consultation, assessment, and review utilizing endorsed assessment tools.
  • Participate in policy and procedure initiatives and continuing quality improvement activities that monitor and improve standards of care.
  • Manage the performance appraisal and development of staff.
**Requirements:**- Evidence of vaccination against Measles, Mumps, Rubella, Varicella (Chicken Pox), Pertussis (Whooping Cough), Hepatitis B, and COVID-19.- Tuberculosis screening to identify if the tuberculosis vaccination may be required.- A current Aged Care check.- Driver Licence.**About Us:**Our community-based program delivers in-home care to people living across the Gold Coast region.
