About the Role

The Residential In Reach (RIR) NP will be responsible for providing expert care, collaborative planning, empowerment through education, community connections, and adaptation to meet the needs of our service.

Key Responsibilities

  • Provide comprehensive assessments, diagnoses, and tailored treatment options directly to patients.
  • Work with multidisciplinary teams to create personalised care plans that prioritise patient goals and preferences.
  • Offer health education and support to patients, families, and care staff, ensuring optimal outcomes and understanding of care processes.
  • Build strong partnerships with GPs, residential care staff, and community services to manage patient care effectively.
  • Evaluate practice and lead in professional development and leadership within the MATS team.

Requirements

  • Current endorsement by the Australian Health Practitioner Regulation Agency as a Nurse Practitioner.
  • Masters of Nurse Practitioner (or a Masters granted equivalence of qualification by the Australian Health Practitioner Regulation Agency plus an approved Therapeutic Medication Management module) or working towards completion.
  • Evidence of competent utilisation of extensions to advanced nursing practice according to approved Clinical Practice Guidelines in the clinical area.
  • Advanced interpersonal, communication (written and verbal) and problem-solving skills.
  • Knowledge, understanding and experience in research and quality improvement.
