Job Description

The role is responsible for providing high-quality, responsive, and person-centred support to assist participants enhance their everyday living skills, gain access and participation in community activities, and improve their quality of life and physical and mental health.

About the Position

This position requires working with external care teams and families to provide comprehensive support to individuals living with complex trauma and psycho-social challenges. The focus is on meeting individual needs and goals, enhancing daily living skills, and promoting community inclusion.

Responsibilities:

  • Support daily living skills and community activities to meet individual needs, including housekeeping, personal hygiene, administering medication, and transportation to appointments and community events.
  • Maintain up-to-date knowledge of participants' needs and care plans, including changes to care requirements.
  • Foster community inclusion and participation dependent on individual needs, abilities, and preferences, supporting participants to establish and maintain relationships and interests through access to community activities and services.
  • Collaborate with clinical teams, attend training sessions, and regular updates to ensure treatment plans are met and implemented to a high standard of care.

Requirements and Qualifications

To be successful in this role, you should possess the following experience and qualifications:

  • Qualification in disability, mental health, or related field (desirable)
  • 2 years' experience working with individuals with complex mental illness, providing behavioural support
  • Demonstrated ability to support individuals with high needs
  • A clear understanding of professional boundaries and restrictive practices
  • Effective written and verbal communication skills, with the ability to write clear case notes
  • Strong interpersonal skills to collaborate as part of a diverse team and work autonomously
  • NDIS Worker Screening Clearance
  • Current First Aid Certificate
