Job Summary:
Integratedliving is seeking a highly skilled and dedicated Clinical Nursing Professional to join our dynamic team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and promoting health and wellness in our community.
Key Responsibilities:
- Provide health promotion and education to clients and carers
- Promote and deliver our award-winning Wellness for Independence programs in person and virtually
- Monitor and promote health outcomes via Telehealth
- Provide direct care services across a range of different funding programs, including wound care, diabetes care, medication management, stoma care, and catheter care
- Assess, develop, and evaluate nursing care for all clients through a diverse and collaborative approach
- Work with clients to develop and achieve client care plans that are goal-driven and client-centered
- Assist clients to become independent with managing health needs through a Wellness and Reablement approach to achieve levels of function in conjunction with Allied Health clinicians
- Understand community nursing and the principles of primary health care
Requirements:
- Division One Registered Nurse with the Australian Health Practitioners' Regulation Authority (AHPRA)
