Compassionate Home Care Nurse

Job Responsibilities- Assess and document patients' needs, making relevant referrals as necessary.
- Deliver individualised care plans that promote independence and improve quality of life.
- Liaise closely with multidisciplinary teams to ensure seamless care coordination.
- Observe, record and report any changes in patients' physical or mental conditions.

Requirements- A valid State Drivers Licence.
- Registration with the Nursing & Midwifery Board of Australia (AHPRA).
- Minimum three years post-graduate experience in acute nursing for Registered Nurses.
