Job Opportunity

We are seeking a Wellness and Lifestyle Coordinator to join our team at Oakfield Lodge.

Key Responsibilities:
  • Foster a culture of wellness and lifestyle services that empower residents to engage in meaningful activities, enhancing their quality of life.
  • Coordinate and facilitate direct reports, ensuring the delivery of person-centered safe and effective wellness and lifestyle programs.
  • Support residents in maximizing their independence, promoting their health, wellness, and quality of life.
  • Implement best practice and evidence-based tools to plan and deliver high-quality lifestyle and exercise services, focusing on person-centered care and healthy aging through goal setting, emotional support, and health literacy.
  • Promote participation in community and facility activities tailored to each resident's needs.
  • Collaborate with the Early Intervention team to identify and develop Recovery Pathways for residents at risk of decline and frailty.
  • Ensure residential lifestyle programs operate in accordance with Southern Cross Care procedures.
  • Drive organizational projects to improve client outcomes as directed by the central Wellness and Lifestyle team.
Requirements
  • Ability to facilitate and coordinate direct reports.
  • Knowledge of best practice and evidence-based tools for wellness and lifestyle services.
  • Skills in promoting health, wellness, and quality of life.
  • Capacity to implement organizational projects.
