The Role

As a Case Manager, your responsibilities will include:

  • Delivering customized support to at-risk individuals and families.
  • Collaborating with clients, families, caregivers, and service providers.
  • Evaluating needs, formulating care plans, and tracking progress.
  • Managing a caseload of youth in therapeutic settings.

Essential Qualifications:

  • A diploma in Community Services or a degree in psychology, social work, or a related field.
  • Experience in supporting adolescents and families dealing with trauma.
  • Understanding of intergenerational trauma and applicable legislation.
