Care Services Coordinator Role

We are seeking a dedicated and compassionate Care Services Coordinator to join our team. The successful candidate will be responsible for delivering high-quality, person-centred care services that enhance the lives of our residents.

Key Responsibilities:
  • Comprehensive Clinical Assessments: Conduct thorough assessments of residents to identify their needs and develop individualised care plans.
  • Collaboration and Communication: Work closely with staff, residents, and families to achieve outstanding care outcomes.
  • Relationship Building: Foster strong relationships with residents and their families through regular communication and support.
  • Leadership and Support: Lead and support care staff in delivering exceptional care services, promoting a culture of excellence and teamwork.
  • Meds Management: Manage medication according to resident needs and legislative requirements, ensuring safety and compliance.
  • Operational Duties: Contribute to operational duties and continuous improvement initiatives within the facility, driving quality and efficiency.
Requirements:
  • AHPRA Registration: Hold current AHPRA registration as a Registered Nurse with a minimum of 2 years' experience in a similar role.
  • Leadership and Communication: Demonstrate excellent leadership skills, verbal and written communication abilities, and a genuine passion for caring for the elderly.
  • Computer Skills: Be proficient in computer systems, with a strong understanding of software applications relevant to aged care.
  • WHS Understanding: Possess a solid understanding of WHS responsibilities, including manual handling principles, to ensure a safe working environment.
  • Legislative Knowledge: Have knowledge of Aged Care Legislation, Accreditation, and AN-ACC processes, which is highly regarded.
  • Availability: Be available to work full-time hours, with flexibility to meet the changing needs of the facility.
  • Credentials: Hold a valid police check clearance and full Australian Working rights.
